Care Coordination Program Information


Area Agency on Aging, Region 9 (AAA9) offers the Care Coordination Program (CCP) as part of a consumer-centered, coordinated, comprehensive network of community-based services. The purpose of the Care Coordination Program is to facilitate access and utilization of home and community-based services for older persons and their caregivers to age in place safely and support them as their needs change.

Consumers will be assigned a Case Manager who will monitor the delivery of all services, periodically reevaluate the consumer’s needs, and assist the consumer in connecting with community resources. Care Coordination is provided at no charge. A “sliding fee scale", based on income, gives the consumer an opportunity to make a voluntary contribution towards the cost of their services. Services are limited to 12 months and based on a first-come, first-served basis, contingent upon program funding and availability.
