The Area Agency on Aging Region 9, Inc. (AAA9), located near Cambridge OH, is interviewing for PASSPORT Case Managers. This position is responsible for providing case management for clients in the PASSPORT Home Care program. Applicants must be either a Licensed Social Worker (LSW) or a Registered Nurse (RN) to be eligible for this position. One year of experience in home health care, medical social work or geriatrics, MRDD, or mental health is required. Ability to travel independently for consumer home visits required. Starting salary for this position is $34,439 with annual step increases the first four years of employment. AAA9 also provides an excellent benefits package including health and dental insurance, life insurance, 403(B) retirement package, flexible hours, sick time, personal days, and paid vacation.
